Open Source FHIR Projects

There are some fantastic open source FHIR projects out there.

Well maintained, rich in features, each with their own focus and areas of specialty.

Here are five that are well worth investing time and resources in. You might have heard of one or two — I doubt you’re familiar with all five.

1. b.well FHIR server

A well maintained FHIR server backed by MongoDB. It implements GraphQL, a unique Bundle $merge operation, as well as optimistic concurrency support. A hidden gem!

2. Medplum

More than just a FHIR API, this project includes a CDR, an SDK, and a React based UI component library. If you’re building a healthcare app with FHIR at the center this is a fantastic and little-talked-about foundation.

3. Metriport

A host of tools built into a single project, including C-CDA to FHIR conversion, PDF conversion, and a FHIR explorer. Metriport are relatively new on the scene and this project hasn’t had much exposure.

4. SQL on FHIR (v2.0)

One of the most advanced community projects in the FHIR space, SQL on FHIR is all about flattening out your FHIR data and making it accessible to SQL type queries. It fixes a problem every FHIR project faces eventually — how to run big analytical queries.

5. Azure Health Data Services Toolkit

My favorite open source project from Microsoft — a complete middleware solution that includes caching, input and output channels, and custom operations. It sits between the consumer and the FHIR server. A requirement for real-world solutions.

There’s is a mix of MIT, Apache and GPL licenses in the above projects, so make sure you’re happy with the licensing before you invest time and resources.
