It’s surprisingly difficult to find a list of FHIR server providers.
The list below was updated in August 2025 and includes all the providers I’m aware of as well as all of the open source FHIR servers. A recent addition is the “New Entrants” section where I list providers that I am unfamiliar with. They may be new companies working with FHIR or established companies with a lesser known FHIR offering.
I created four categories and where possible assigned one or more category to each server provider.
cloud
Cloud hosted, usually managed for you but sometimes managed yourself in a VM.
managed
“off the shelf” servers managed for you, usually without access to the underlying database.
on-prem
You buy the server and host it yourself. Hosting can be a local installation but is usually containerized.
OS
Free and open source. You build and maintain the server yourself. Often needs to be run inside a container, either locally or in the cloud.
Servers are listed in no particular order, so don’t assume ranking where none is intended.
And remember, not all FHIR servers are equal. Do your research!
